Gentle Hearts Living Solutions is a registered nurse owned senior placement and referral agency with a highly developed capacity for the unique assessment needed to evaluate an individual person and their needs. We have an expert knowledge of the rapidly expanding and increasingly regulated healthcare industry with a focus on residential care. With over four decades of experience in multiple facets of the healthcare system, our compassion and expertise have benefited many families in the area of respite and permanent placement within residential facilities. The assessment process focuses on an individuals' level of care needs, location, physical, emotional and social needs, as well as budget.
Research
Our agents utilize an expansive, active professional network to find the perfect placement fit for you or your loved one. This can include independent living, memory care, assisted living , skilled nursing facilities, and assisted living homes. We will work diligently to prepare options that fit your defined assessment parameters, keeping you updated each step of the way. Our agents will tour the facility with you to meet with the owners, staff, and managers to ensure a proper fit. Let us provide expert guidance, education, and navigation to find the ideal setting!
Transition
Once we have found that perfect place to call home, our agents will work with family and medical staff on a seamless transition to the new setting. We will assist in the preparation of any needed documents and help fulfill requirements set forth by the Arizona Department of Health Services. We can also connect you with many resources such as transportation, medical equipment, and community services such as mobile physicians, home health, and palliative care or hospice agencies.
