Are there any costs associated with your service?
The client and family are never charged for assessments or placement services. Fees are paid by the Assisted Living Home or Facility management. This is outlined in a Referral Fee Disclosure document that is signed with each consultation as directed by the Arizona Department of Health Services (AZDHS).
Do you have any affiliations with any of the facilities you work with?
Gentle Hearts Living Solutions does not hold any stock in any facilities. This is also outlined in the Fee Disclosure document. Any Senior Referral Specialist is required by AZDHS guidelines to disclose if they have any vested interest in a facility or assisted living home they work with.
Will we get to keep our same doctors?
You can continue to see your doctors as you wish. If you are finding it difficult to get out of the house, each facility does have the ability to have a mobile physician that will come to the home. There are several of these services across Maricopa County. Referrals can be made by Gentle Hearts Living Solutions.
